Community Integration and Habilitation
What is Community Integration and Habilitation?
Community Integration and Habilitation (CIH) provides services that allow individuals to learn necessary skills either at home or in a community-based setting to help them gain self-independence in their home and community.
How can someone be eligible for this service?
- Be diagnosed with an intellectual disability before age 22
- Meet ICF/IID level of care
- Must show significant limitation in three of the six categories: self-care, learning, self-direction, capacity to live independently, speaking ability, and mobility

- Structured Family Caregiving- Level 1-DDRS
- Structured Family Caregiving- Level 2-DDRS

Family Supports Waiver
The Family Supports Waiver is a program made to help individuals with disabilities acquire limited non-residential support services that will help them adapt to their environments.
When deciding to go for the Family Supports Waiver program, one can expect the following:
- Community-Based Habilitation
- Facility-Based Support
- Facility-Based Habilitation and Support
- Respite – General
Money Follows Person – CIH Transfer
The money follows the person program (MFP) is designed to help individuals who were in institutional settings go back to their homes or into the homes of their loved ones.
Individuals can only become eligible for this program if:
- They are a resident of a qualifying institution for at least 90 straight days.
- Their needs can be safely met through services available in the community.
- They are a recipient of an Indiana Medicaid at least one day prior to discharge.
- They meet minimum requirements for a funding source partnered with Indiana’s MFP program.
Once you are granted this program, you can expect the following:
- Community-Based Habilitation
- Facility-Based Support
- Facility-Based Habilitation and Support
- Respite – General
- Structured Family Caregiving- Level 1-DDRS
- Structured Family Caregiving- Level 2-DDRS
- Structured Family Caregiving- Level 3-DDRS
Wellness Coordination
Our wellness coordination services will help develop, maintain, and monitor your loved ones’ wellness coordination plan and medical services that are required to help address their needs.
This type of service will extend beyond the routine doctor visits under the Medicaid state plan and are specifically designed for individuals who need the assistance of an RN or LPN to properly coordinate their medical needs. The following tiers of support have the following requirements:
- Tier 1: Healthcare needs require a face-to-face visit once a month with an RN or LPN and consultations/reviews based on the individual’s current healthcare needs.
- Tier 2: Healthcare needs require a face-to-face visit twice a month with an RN or LPN and consultations/reviews based on the individual’s current healthcare needs.
- Tier 3: Healthcare needs require a face-to-face visit once a week with an RN or LPN and consultations/reviews based on the individual’s current healthcare needs.
